printing a book from a PDF file?

I have a pdf file which is a few hundred pages long. I would like to have it printed and bound as a book. (not that spiral bound option kinkos would do). I’ve been investigating various print on demand options, but all of them are geared to book authors who plan on selling many copies of their book, and charge a few hundred dollars for setup. In my case, I plan on printing only one copy, and am hoping to get away for about . Any ideas on where I can do this?
was really just an estimate - if it costs or I don’t care too much, I just don’t want to pay several hundred dollars for professional services when I only plan on making myself one copy of the book.

    you can do it on http://www.lulu.com, but not for $10. If you do all of the formatting yourself, you will pay only the printing and shipping costs.

    You can put the file on a disk and take it to kinko’s and get it spiral bound, but even that may cost more than $10 for a "few hundred" pages.

    I regularly do a spiral bound of 45 pages via lulu.com and that costs more than $10. A hardcover, perfect bound of some hundreds of pages will cost a fair amount.

    I think your pricing goal may have to be adjusted to be a bit more realistic.

    Submit it for NaNoWriMo. If you get 50,000 words–well, they did this last year at least–Amazon will print a free hardcover copy for you and mail it to you.
